Chattanooga / Southeast TN Continuum of Care

A Continuum of Care is the group organized to carry out the responsibilities prescribed by the U.S. Department of Housing and Urban Development’s CoC Program Interim Rule for a defined geographic area designed to:

  1. Promote communitywide commitment to the goal of ending homelessness

  2. Provide funding for efforts by nonprofit providers, States, and local governments to quickly rehouse homeless individuals and families

  3. Promote access to and effective utilization of mainstream programs by homeless individuals and families

  4. Optimize self-sufficiency among individuals and families experiencing homelessness

The Chattanooga / Southeast TN CoC, or TN-500 as named by HUD, covers an 11-county region and is comprised of all agencies and organizations providing services to those experiencing homelessness in the region. Those 11 counties are Bledsoe, Bradley, Franklin, Grundy, Hamilton, Marion, McMinn, Meigs, Polk, Rhea, and Sequatchie.

CRHC’s Role

  • The Collaborative Applicant is the eligible applicant designated by the CoC to collect and submit the:

    • CoC Registration

    • CoC Consolidated Application

    • CoC Planning Grant

    And host the annual CoC Competition

  • Responsibilities:

    • System Administration and Operation

    • End User Training

    • Reporting Requirements

    • Quality Assurance

    • Supporting CoC Goals

    • Resource Allocation

  • Implement Coordinated Entry System to equitably distribute housing resources to the most vulnerable persons experiencing homelessness

How it works.

Access

Household encounters service agency, case manager, street outreach, etc.

Prioritization

Household is placed on By-Name-List (BNL) and triaged / prioritized according to vulnerability

Assessment

Universal Housing Assessment (UHA) is completed with household

Referral

CRHC refers household to appropriate program according to availability and eligibilty
